Nurse's Notes

REQUIREMENTS FOR 6TH GRADERS:

1. Certificate of Child Health Exam - done within one year prior to the first day of school, complete with record of current required immunizations.
Immunizations specific to 6th graders are 1 dose of the TDAP vaccine and 1 dose of the Meningococcal Conjugate vaccine.
A second Varicella vaccine is required if it has not been given.
Due Date: October 15th 2020.
